Community Care Program (CCP)

Rincon Family Services’ Community Care Program (CCP) provides high-quality, non-medical in-home support to older adults through trained and supervised Home Care Aides. Our caregivers receive specialized training to deliver compassionate, reliable, and person-centered care.

We believe caregiving is more than assistance—it’s an opportunity to build meaningful connections. Our services are designed to transform daily routines into engaging interactions that promote dignity, independence, and overall well-being. By supporting clients with everyday tasks, we help older adults remain safe and comfortably in their own homes while maintaining their quality of life.

Main Services Provided by Our Home Care Aides

• Basic housekeeping (making and changing beds, dusting, washing dishes, vacuuming, floor cleaning, kitchen and bathroom upkeep, and laundry)
• Light home maintenance
• Assistance with essential shopping (groceries and medication pick-up)
• Meal planning and preparation
• Medication reminders (self-administered medications only)
• Monitoring clients’ condition and reporting changes
• Assistance with non-medical personal care (bathing, grooming, toileting, dressing, and hygiene support)

Eligibility Requirements
To qualify for the program, individuals must:

• Be 60 years of age or older
• Be assessed as physically in need of services (at least moderately impaired)
• Be a resident of the State of Illinois
• Meet asset eligibility requirements
• Apply for Medicaid (Medical benefits)

Violence Prevention Convener
