Substance Use & Recovery Support

We offer integrated behavioral health services to support individuals living with substance use concerns. Our team provides counseling, relapse prevention planning, and connection to community resources — all within a recovery-affirming, non-judgmental environment.

Grief & Loss Counseling

Losing someone or something important is one of life's most difficult experiences. Our compassionate counselors provide support to help individuals process grief related to death, separation, or significant life transitions — at your own pace.

Psychiatric Evaluation & Medication Management

Available for individuals ages 12 and older. Our psychiatric team provides comprehensive diagnostic evaluations, medication prescriptions when appropriate, and ongoing monitoring to support your treatment goals.

Intensive Case Management

For individuals with complex mental health and social service needs, our case managers provide coordinated care, advocacy, and navigation support across multiple systems — so nothing falls through the cracks.

590 Crisis Care System

Help Is Available — No Insurance Required

If you or someone you know is experiencing a mental health crisis, help is available right now — at no cost to you. The 590 Crisis Care System is a statewide initiative administered by the Illinois Department of Human Services (IDHS) Division of Mental Health. Rincon Family Services is a proud partner in this program, bringing crisis support directly to our community

When a crisis occurs, our trained team responds directly to where you are. You don't need to travel. You don't need to worry about cost. Our team is there to stabilize, support, and connect you with ongoing care.

Wellness West Program

Supporting Your Recovery — Every Step of the Way

Leaving the hospital can feel overwhelming especially when you're facing housing instability, financial stress, or a lack of a support system. The Wellness West Program is here to change that.

Launched in 2023, Wellness West bridges the gap between hospital discharge and community care so you never feel like you're starting over alone.

How We Help

Our team of three dedicated case managers works directly with local hospital partners including Rush Medical Center, Mount Sinai Hospital, and Humboldt Park Health to coordinate a smooth, supported transition home.

  • Rapid Response: Crisis Response
    We respond quickly to hospital referrals for patients in urgent situations.
  • Staying Connected: Phone Access:
    In partnership with the Medical Home Network (MHN), we connect patients without reliable phone access to mobile devices keeping them in contact with care providers and support systems.
  • Ongoing Outreach: Community Outreach:
    We maintain active relationships with hospital partners through on-site visits and community events, ensuring strong coordination and faster support for clients.
