About our Community Care Program (CCP)

Our CCP Program provides general non-medical support by supervised home aides who have received specialized training in the provision of home care aide services. Rincon’s in-home care services are built around transforming day-to-day caregiving into opportunities for meaningful conversation and activities that engage and enrich the lives of clients. This means that Homecare Aide service helps older people maintain their quality of life by continuing to live in their own homes, by helping them with some of the tasks they can no longer complete on their own.

Main Services Provided by Our Homecare Aides:

  • Basic housekeeping tasks (e.g., making and changing beds, dusting,
    washing dishes, vacuuming, cleaning and waxing floors, keeping the
    kitchen and bathroom clean and laundry.
  • Light home maintenance
    Assisting with essential shopping (basic groceries and pick up
    medications).
  • Meal planning and preparation.
  • Assisting with self-administered medication, which shall be limited to:
    reminding the client to take his/her medications and reading instructions for
    utilization.
  • Observing the client’s functioning and condition and reporting changes.
    Assisting with personal care tasks that are not medical in nature, such as:
    showering/bathing, shaving, hair shampooing, toileting, dressing or similar
    tasks.

Eligibility Requirements:

  • Age 60 or older
  • Determined to be moderately physically impaired
  • Resident of the State of Illinois
  • Meets the asset requirements
  • Applied for Medical benefits

  1. After the initial personal information intakes, an IDoA Case Manager will work with the client to determine their needs and create a Plan of Care.
  2. The client chooses their in-home services provider, such as Rincon Family Services
